Report Your Case Status

 

603 Legal Aid relies on grants in order to fund all of our activities.  In exchange, our funders require we report on certain aspects of the cases we place with volunteers.  To streamline the process, we have made these forms available online.  All forms related to reporting information on your cases can be found here.

Initial Case Disposition forms should be submitted after your first interview with the client or 30 days after a missed appointment (if no further contact has been had with the client)

Quarterly Case Status Update should be submitted no later than two weeks after the close of each quarter (by April 15, July 15, Oct 15 and Jan 15) for as long as you have the case open. This form allows us to record work being done on these cases for reporting to our funders.

Final Case Disposition Form should be submitted within 30 days of closing your case. Please attach any final orders and your timesheet when you submit this form.

Expense Reimbursement should be submitted as expenses are incurred. This form can also be submitted at the time the final case disposition form is submitted.  This form should be used by any volunteer seeking reimbursement of expenses, including volunteers with DOVE or LITP. 

DOVE Case Disposition Form should be submitted within two weeks of closing the case (i.e. attending the Final Protective Order Hearing) or if the client has failed to contact you within 30 days of your agreement to take the case.

LITP Case Update Form should be submitted quarterly for as long as you have the case open. The grant which funds this Project requires reporting on the amount of time our volunteers spend on cases as well as how the case is proceeding. Underreporting our volunteer hours in these cases can have a detrimental effect on our funding.
