DOVE Case Disposition Form

DOVE Project volunteers should complete the Disposition Form and submit it within two weeks of the case closing.

Volunteers with the DOVE Project should complete the DOVE Case Disposition Form below and submit it within two weeks of the case closing (i.e., final hearing) or if you have no contact with the client. In cases where the client fails to contact you, please allow 30 days for the client to make contact before submitting the Disposition Form.

If you discover that you cannot represent this client, please notify the crisis center and the DOVE Project immediately by emailing or calling the Project Coordinator at (603) 584-4130.
